PERMISSION FORM
In order to protect everyone’s health and safety, we will ask all participants at each activity session to answer some Covid-19 screening questions. If you do not give permission for this, please do not send your child to our activity session. Please note that information will be shared with Public Health officials if there is a confirmed case of Covid-19.
All Information provided is confidential and will only be viewed by staff employed by InUnity and direct partners to the project you are attending. In line with GDPR procedures, we only ask for information that is relevant to this project and all data will be stored in line with data protection policy. Your personal data will not be shared with anyone outside our organisation. Please provide as much information as possible.
